SWIM Flap Breast Reconstruction at a Glance
SWIM flap reconstruction was developed as an alternative to the traditional options available to mastectomy complex reconstruction or complete removal of the breast without reconstruction. It is an evolved version of the Goldilocks mastectomy, which was pioneered by Dr. Heather Richardson, one of our esteemed partners at the Bedford Breast Center. The Goldilocks method maintained a small breast shape without implants, but it did not preserve the nipple.
Collaboration between Dr. Richardson and our own Dr. Lisa Cassileth produced the SWIM flap reconstruction technique, which preserves the nipple. The result looks more like a small sized-breast reduction, although it is, in fact, a complete nipple-sparing mastectomy.

In Swim flap reconstruction, the mastectomy and breast reconstruction are performed in one surgery. During the mastectomy, the breast surgeon precisely excises the breast tissue, while preserving the healthy skin and subcutaneous fat. Next, the plastic surgeon meticulously shapes and sculpts the preserved tissue, creating a natural breast shape, with the nipple intact.
There is no tissue transfer from other areas like the abdomen, back, or buttocks. The outcome is a breast that is smaller but similar in shape to your natural breast. The incisions from SWIM flap reconstruction are similar to those of a breast reconstruction.
SWIM flap breast reconstruction uses the patient's own breast skin and fat leftover following the mastectomy to recreate a new, smaller breast mound.

The SWIM flap method can also be a good fit for patients who have certain health issues that preclude other forms of reconstruction.
Recovery can take several weeks to months, depending on the extent of the surgery and your individual healing process. Normal side effects include bruising, swelling, and discomfort. With proper care and attention, the healing process progresses smoothly, and most patients return to work within two weeks. Your surgeon will provide pain medication and may have you wear compression garments to help with swelling during the initial healing period.
Other instructions include refraining from strenuous exercise and heavy lifting for several weeks. The doctor will examine you and check your healing at follow-up visits. It’s very important to follow aftercare instructions, attend all of your post-operative appointments, and contact the after care team any time you have questions or concerns. Early recovery will typically take a few weeks, while full healing can take several months. As you gradually return to various activities, you will be able to enjoy the full results of your procedure—a naturally restored breast shape and preserved nipples without the ongoing maintenance of breast implants.
The overall procedure time depends on the degree of reconstruction needed and the amount of tissue used to preserve the nipple. Surgery typically takes several hours.
We generally advise patients to spend a few days at an aftercare facility to receive nursing care and guidance during early recovery. After returning home, patients should take several weeks to rest. Light exercise is usually okay after about two weeks, but you will need to avoid heavy lifting for six weeks. We recommend that out-of-town patients stay in the Los Angeles area for at least ten days after surgery.
